For information and applications you may reach the U. S. State Department’s Passport Services website. This website will give you all the information you need to apply for a passport. If you have any questions, you may call the Town Clerk’s Office at 978-640-4355.

Applicant: DO NOT sign passport application until you are in front of passport agent.
IDENTIFICATION:
Certified Birth Certificate (Raised Seal & Date of Record)
Previously issued Passport may be used in lieu of Birth Certificate
(Applicants under Age 18 – are required to submit a birth certificate & previous passport)
Social Security # required on form
Valid Drivers License

2 Passport Colored Photos
2 X 2 Full Face
Will fit into passport picture framework

FEES – AGE 15 YEARS AND YOUNGER

AGE 14 THROUGH 17:
- Must be present AND personally sign passport application
- Parents consent is required
- Must present acceptable identification
- Both parents must sign application or give written consent
- Inconvenience is not a mitigating factor
UNDER AGE 14:
- Must be present AND requires both parents’ signatures on passport application.
EXPEDITED SERVICE:
Two (2) week turn-around
Additional $60.00 per passport application - Payable to the U. S. DEPT. OF STATE
ADULTS: $75.00 FEE + $60.00 = $135.00
AGE 15 AND YOUNGER: $60.00 FEE + $60.00 = $120.00
